Stability Post Care (SPC)

What is Stability Post Care?

Stability Post Care (SPC) is a funded support package designed to assist young people aged 18 to 21 years who are transitioning from care into independent adulthood. This period can be challenging, and SPC recognises that young people leaving care often require additional support to build stability, confidence, and life skills as they establish their independence.

The core aim of SPC is to empower young people to become self-reliant, while ensuring they are not left without support during times of transition or uncertainty. Support is flexible, strengths-based, and responsive to each individual’s circumstances, goals, and pace of development.

How SPC Supports Young People

SPC provides tailored case management and practical assistance to support young people across key areas of life, including:

  • Housing and tenancy support
    Assistance to secure and maintain stable accommodation, understand tenancy responsibilities, manage rent and utilities, and respond to challenges that may place a tenancy at risk.
  • Independent living skills
    Support to build everyday skills such as budgeting, cooking, cleaning, time management, and navigating services, helping young people feel confident living independently.
  • Education, training and employment
    Guidance and advocacy to access education, vocational training, employment pathways, and career planning, supporting young people to work toward long-term economic independence.
  • Health and wellbeing
    Support to access health services, mental health supports, and wellbeing resources, recognising the importance of holistic care that includes physical, emotional, social, and cultural wellbeing.
  • Connections and community
    Encouragement to strengthen positive relationships with family, kin, community, culture, and support networks, helping young people build a sense of belonging and identity beyond care.

A Flexible, Individualised Approach

SPC funding is flexible and can be used n ways that best support a young person’s individual needs and goals. Support levels can increase or decrease over time, allowing young people to build independence while knowing assistance is available when required.

 

Empowering Young People for Independence

The program is grounded in a person-centred approach that recognises each young person as an individual with their own strengths, goals, and experiences. Young people are actively involved in decisions about their lives, ensuring their voice is heard and respected. Support plans are developed collaboratively, focusing on what matters most to the young person while promoting independence, confidence, and practical life skills.

This approach also acknowledges the importance of identity, culture, and connection. Support is delivered in a way that respects each young person’s background and encourages strong links to family, community, and culture where appropriate. Flexible and responsive, the program adapts to changing needs, empowering young people to make informed choices and move toward a stable and independent future.
